Cardiac Rehabilitation Program

Welcome to the All Care Medical Consultants Cardiac Rehabilitation program. We are so excited for you to start the program with us. As you begin this journey, we have created educational materials to support you on your way. Our program is preventative, which means you do not need to have multiple “qualifying diagnoses.” It also means that you may have graduated from a previous Cardiac rehabilitation program and no longer qualify traditionally. As a value-based internal medicine practice who believes in treating all aspects of “you,” our practice will provide you with education and support through the lens of lifestyle medicine. “Lifestyle medicine is a medical apporach that uses evidence-based behavioral interventions to prevent, treat and manage chronic disease.” – American College of Lifestyle Medicine.

What is Cardiac Rehabilitation?

Cardiac rehabilitation is a medically supervised program designed to improve your heart health after experiencing heart problems or treatments. It can take place at an outpatient clinic or in a hospital rehabilitation center. At All Care, your cardiac rehab team includes doctors, nurse practitioners, exercise physiologists, nutrition and weight management specialists, licensed mental health counselors, in addition to other medical and health care providers. Our goal is to help you make lifestyle choices to improve your heart health.

Who needs Cardiac rehab?

You may benefit from cardiac rehab if you have had a:

  • Heart attack in the last 12 months
  • Heart condition, such as coronary artery disease (CAD), stable angina, or heart failure
  • Peripheral artery disease
  • Heart procedure or surgery, including:
    • Coronary artery bypass graft (CABG) surgery
    • Angioplasty and stenting
    • Heart valve repair or replacement
    • Pacemaker or implantable cardioverter defibrillator (ICD)
    • Heart transplant
Program Expectations...

You will participate in a group exercise setting and receive assistance in making lifestyle changes to support your heart health. A typical cardiac rehab consists of 36 sessions over 12 weeks, but it can vary depending on your condition, the program, or your doctor’s recommendations. At All Care, you are offered 24 sessions over 8 weeks. After the completion of the 8 week program, your healthcare team may recommend additional sessions.

During rehab, you will:

  • Have a medical evaluation to figure out your needs and limitations. The medical staff then tailors a program to your needs
  • Exercise on a treadmill, recumbent elliptical, rowing machine, or other form of aerobic exercise
  • Be monitored by an exercise physiologist and/or health and wellness care team for a change in symptoms
  • Follow safe physical activity programs that gradually improve your strength
  • Slowly move more into a more intensive program that lets you work longer and harder
  • Begin strength training as prescribed
  • Have heart rate, blood pressure, and EKG monitored

The goals of this program are to improve your heart health and overall lifestyle. Continue the changes you’ve made after completing our program. Inquire about additional assistance or guidance throughout your program:

  • Wellbeing Through Health Lifestyle Event
    • Every 3rd Wednesday from 12-4 pm
    • Healthy eating education and strategies with a health care provider
    • Stress management strategies through our yoga classes
    • Supervised corrective exercise sessions to address orthopedic injuries or limitations
    • Mindful connections and support with our licensed mental health counselor

What is Pulmonary Rehabilitation?

Pulmonary rehabilitation is a supervised medical program that helps people who have lung diseases live and breathe better. A comprehensive program includes pa- tient assessment and individualized therapies that include, but are not limited to, exercise and breathing training, education, and behavior change. At All Care, your pulm rehab team includes doctors, nurse practitioners, exercise physiologists, nutri- tion and weight management specialists, licensed mental health counselors, in addi- tion to other medical and health care providers. Our goal is to help you make lifestyle choices to improve your respiratory health.

Who needs Pulm rehab?

You may benefit from pulm rehab if you have had a:

  • Cystic fibrosis
  • Chronic bronchitis
  • Asthma
  • Pulmonary hypertension
  • Pulmonary fibrosis
  • Chronic obstructive pulmonary disease (COPD)
  • Shortness of breath and have difficulty performing daily activities despite daily medication use
Program Expectations...

You will participate in a group exercise setting and receive assistance in making lifestyle changes to support your heart health. A typical pulm rehab consists of 2-3 sessions per week over several weeks, but it can vary depending on your condition, the program, or your doctor’s recommendations. At All Care, you are offered 16 sessions over 8 weeks. After the completion of the 8 week program, your healthcare team may recommend additional sessions.

During rehab, you will:

  • Have a medical evaluation to figure out your needs and limitations. The medical staff then tailors a program to your needs
    • Exercise stress test to measure oxygen level, blood pressure, and heart rate, breathing tests, and/or the six-minute walk test
  • Be monitored by an exercise physiologist and/or health and wellness care team for a change in symptoms
  • Exercise via strength, flexibility, and/or aerobic training
  • Learn specific breathing techniques to control breath, avoid running out of breath, and clear mucus from lungs

State-of-the-art equipment with a very modern touch. In the studio you will have all you will need to achieve your goals. Designed by our fitness experts to give our members a simple direction to see physical success. Silver Sneakers is accepted for fitness access in our fitness studio. The All Care Fitness Studio offers classes to maintain your strength, enhance your endurance and improve your flexible. These classes are included with your Fitness Access Pass.

Our Wellness Team Members have very unique educations and background to be there to give you the tools needed to succeed in your fitness journey. Using techniques to keep you safe and injury free, our fitness experts can set you up with a program that will use the method of “smarter not harder” to help you reach your goals in a realistic time frame. Whether you need guidance, motivation or accountability our specialist can work with you one on one or in a small group to help you achieve happiness through fitness.

Our fitness facility is open Monday-Friday from 8:00 am – 5:00 pm. To participate in group exercise classes or work one-on-one with one of our Wellness Team Members, please contact the Wellness Director or a front office associate for more information.
